        Thank you for your interest in participating in the Implementation Planning Workshop. In order to attend the Implementation Planning Workshop, you must complete the following 3-step process.

        • Step 1: Complete the application below and submit it by the application deadline. If you have any questions, please contact us at training@mendezfoundation.org or 800-750-0986.
        • Step 2: Once your application is approved, a registration form will be emailed to you. Complete and submit your registration by the registration deadline to complete the process.
        • Step 3: A confirmation letter will be emailed to you with additional instructions to help you prepare for the workshop. Please wait for a formal confirmation before making any final travel arrangements.
